Below is an empty version of the required immunization form. To download a filled-out version, follow these steps:
- Go to https://wa.myir.net/
- Register as one parent with the same email/phone number provided to your pediatrician. This is important as that is used by the system to match your child's details.
- Under the "My Immunizations" tab, add the family/child details using the "Add New Family Member" option.
- Both parents will need separate accounts, but kids can be listed under either account.
- Under "My Immunizations", click on "Certificate of Immunization" after selecting the appropriate child's name.
- This will generate and download a PDF with all the details filled out.
- Print, sign, and return the filled out form to us.
